About the Role
Are you a qualified, HCPC registered social worker who has successfully completed their Assessed and Supported Year in Employment (ASYE)? NB this applies only to social workers who qualified on or after 2012.

You will need to be motivated and flexible with high professional standards and sound knowledge and experience of current legislation, including the Care Act, the Mental Capacity Act and the NHS Continuing Healthcare National Framework and its application to practice in a social care setting.
You will have the ability to manage and support day to day work in a busy, dynamic team effectively prioritising daily challenges and expectations. You will be confident in making sound decisions that respond flexibly and quickly to changing needs that arise in a high pressure Hospital team environment. You will be able to develop effective partnership relationships with individuals, carers and families and positive relationships with professional health staff both within and outside the Hospital team.
You will need to be confident in undertaking safeguarding enquiries, carry out assessments within the community and working with a diverse group of individuals and partner agencies.
You will be able to communicate in a clear, fluent, concise and jargon-free way and in a courteous, calm and professional manner. You will need to have competent IT skills and be able and willing to learn to use a variety of software systems. Applicants will need to have the willingness and ability to travel across Surrey in accordance with the demands of the work and the job profile.
The role within the acute setting will be critical and the post holder will work alongside both health and social care staff to ensure timely discharge for individuals needing support on discharge. The post holder will build on and develop relationships with health colleagues and local providers and will have excellent relationships with locality colleagues, providers, commissioning, quality assurance and area finance teams.

You will be a car driver with full valid UK driving licence and use of a motor vehicle is essential.
