This role has a starting salary of £26,080 per annum based on a 36 hours per week (pro-rata for part-time staff).

We are excited to be looking to recruit Social Care Assistants within our amazing Adult Social Care teams in North West Surrey.

We currently have vacancies in the following teams:

  • 2 x permanent full-time positions with the Spelthorne Locality Team based at Ashford Office, Stanwell Road, Ashford, TW15 3DU. (Part-time applications will be considered for this role).
  • 1 x permanent full-time position with the Surrey Heath Locality Team based at Surrey Heath House, Knoll Road, Camberley, Surrey, GU15 3HD.

About the Role

Your role will be to assist qualified practitioners with a variety of tasks, including assessing people’s needs and carrying out regular reviews, with frequent direct contact with the public on the phone as well as visiting them in their own home or care setting. You will be the first point of contact between the public and the service, gathering initial information and identifying the need for further assessment, as well as signposting people to other services.

You will work in partnership with individuals, their carers, professional staff in other agencies, service providers and Support Brokers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.

You will have the opportunity to work on the new ‘Discharge to Assess’ model to support vulnerable residents leaving hospital settings in a timely and safe manner and helping them build their confidence to be part of the community. This exciting work will include working within hospitals as part of an integrated workforce of Health and Social Care practitioners with the aim of supporting our residents in their recovery in the community.

About You

In order to be considered for shortlisting, we would love to hear about;

  • Your ability to manage a caseload efficiently, regularly monitoring support plans for individuals and carers.
  • Your ability to build relationships with various stakeholders, colleagues and the public effectively.
  • Your experience, paid or voluntary, of working alongside the public, it doesn’t have to involve visiting people in their homes or in care settings!
  • Your IT skills with a willingness to learn to use a variety of software systems.
