We are pleased to announce two 18-month fixed term contract opportunities for Welfare and Discharge Support Coordinators, sitting within the Intervention Team. These roles are developed with East Surrey Place (Surrey Heartlands Integrated Care System) partners, in partnership with Tandridge District Council and Reigate and Banstead Borough Council.

The successful candidates will be pro-active and ambitious, ideally with a track record of working with and supporting people by identifying and responding to complex needs. Experience of working in a multi-agency approach with local Statutory and Non-statutory agencies to support people is essential, and you will need to be keen to learn quickly and advocate for clients confidently. The successful candidates will work closely with healthcare clinicians and housing teams to reduce their dependency on health and social care to ensure a safe discharge hospitals and on-going support plan. A Full Driving Licence with access to a vehicle is essential.

You will be joining a growing, innovative multi-agency team which supports the successful co-ordination of local statutory and voluntary services. You will participate in an induction training programme, benefit from regular role specific supervision and clinical supervision on taking up the post.
