This is an exciting opportunity to work with people in a person-centered way, proactively seeking to maximize their independence. The post will be based at the Cobham Centre for the Community and will be responsible for helping people with learning disabilities to achieve their ambitions and Support Plan outcomes. This will incorporate social education, self-care, independent living skills and employment opportunities.
Building on the principles of Self Directed Support, Cobham Link aims to;
You will work in partnership with individuals, their carers, families, staff and service providers throughout the year.
Successful applicants will be skilled at communicating and working with people with learning disabilities and have experience of working directly with people with social care needs.
You will need to be confident and competent in working with people with a variety of physical and cognitive impairments. You will need to have skills in assessment, project development and have the ability to work professionally in a multidisciplinary/multi-agency context.
