About the Role

There has never been a more exciting time to join The Surrey Transition Services for young people as we embrace the changes in the Children’s, Adult and Education Legislation. We are undertaking a review of how we work and are currently implementing these changes. We work with a cohort of young people in the 14 – 25 age group.

This is an exciting opportunity to work with young people in a person-centred way, proactively seeking to empower them and maximising their potential for independence.

A key part of this post working with our Social Care, Education and Health partners to identify future needs and to inform the young person, their families and the education service of the changes between child and adult provision. There is a real opportunity to make a difference and support the young person towards their journey in preparing for adulthood.

Requirements for the role

# Hold a professional social work, occupational therapy, nursing or other relevant health qualification having completed any post qualification practice or training requirements.
# Significant post qualifying experience.
# Management experience in a variety of Social Care environments, demonstrating good understanding of both the Health and Social Care agendas.
# Current professional registration with the Health Professionals Council.

# High level of understanding and commitment towards the personalisation agenda and be able to demonstrate an applied knowledge of Adult Social Care and Children’s legislation.
# Demonstrate a strong evidence base of supervising staff.
# Demonstrate a strong knowledge base for the safeguarding of vulnerable adults and be able to show evidence of this.
# An understanding, and commitment to supporting carers and demonstrable commitment to delivering support and management in accordance with the Councils Equality and Diversity Policy.
# A team player, committed to promoting change while supporting and developing staff and their practice.

We are building and developing the skills, knowledge and experience of all of our teams in the Personal Care and Support directorate and would welcome applications from candidates with knowledge and experience of working with people with Learning, Physical and sensory Disabilities, who can demonstrate an ability to build further knowledge and skills across other specialism’s within Adult Social Care teams.
