St Peters Hospital, Social Care Hospital Team, is a busy acute hospital team that currently has a vacancy for a Senior Practitioner Operational Lead. The team works closely with the Hospital Trust to ensure patients are discharged appropriately and in a timely way working closely with the individual and their families hospital clinicians, community health and social care teams and providers. The post holder will be part of the management team and will work closely with the Hospital Trust and Social Care Practitioners. It is an exciting time within the hospital with innovative changes being implemented focussing on Whole System Changes with and Community facing teams. This will include the development of greater access for patients promoting an 8:00am till 8:00pm, seven days a week offer to patients from the Social Care Team.
You will need to have had significant post qualifying experience, with some degree of management experience and ideally have worked within both Health and Social Care environments demonstrating good understanding of both the Health and Social Care agendas. The post holder will need to be able to work under their own initiative. They will be an excellent communicator and demonstrate skills and ability to manage and support the day to day running of a busy, dynamic team prioritising daily changes and expectations, be confident, make decisions that respond flexibly and quickly to changing circumstances and the demands of an acute hospital.
You will be strong team player, committed to promoting change and supporting staff and should be able to demonstrate a strong evidence base of supervising staff.You will also have significant knowledge and understanding of older people including Dementia.
