We have vacancies for Community Support Workers in Spelthorne and Staines Social Care team and also for Weybridge Social Care team, this post being based at Walton community hospital.

You will provide in-depth information, advice, guidance and support to service users and their carer, whilst supporting Care Managers and Occupational Therapists in maximising independence.

The position is a primarily office-based role that involves visiting service users and their carers in the community. It is the ideal first step towards a good understanding of the care management process, and will require good general education and strong IT skills. A full driving licence and regular use of car is essential. Some experience in a health or social care environment would be a huge advantage, but is not essential as we provide comprehensive induction and ongoing training.

The post at Walton hospital will involve working in a multi-disciplinary setting to ensure safe hospital discharges.

An enhanced Criminal records Bureau disclosure, POVA and POCA check will be required for this role.
